FastMed Urgent Care offers Back-to-School Discounted $15 Sports Physicals at Select Locations
Clayton, NC – September 2 – FastMed Urgent Care, North Carolina’s largest urgent care network, is offering a Pre-participation Physical Examination (PPE) discount voucher of $15 at select locations to encourage student athletes to get their required physical. In addition, FastMed Urgent Care has been named the official urgent care vendor for the North Carolina High School Athletic Association (NCHSAA), which provides governance and leadership for interscholastic athletic programs and sets the standard for the PPE.
The pre-participation physical examination is required before a student participates in any sport in North Carolina. The exam consists of the physical exam and medical history and must be completed by a physician or physician assistant. All FastMed clinics offer fast, walk-in PPEs but for a limited time select FastMed clinics are offering a voucher for $15 PPEs good through September 30, 2014. FastMed asks student athletes to bring the following information to complete the PPE:
- Current immunization records - Immunization record card with most recent vaccines, dates given, and provider
- Family health history – Any chronic medical illnesses such as diabetes, asthma, heart arrhythmia or murmurs, epilepsy, kidney problems, or seizures that may impact sports participation
- Injury history - Prior personal injuries including surgeries or hospitalization, concussions, fractured or broken bones, repeated swelling joints or dislocations
- Medications list – Any current medications
- Corrective lenses – Corrective lenses for eyes
- Exam Form - A copy of the official NCHSAA Pre-participation Sports Physical Medical Examination Form. To download the form, click here
- A Parent - Minors must be accompanied by an adult chaperone or guardian and have parental/guardian permission
FastMed’s urgent care clinics provide physical examinations 365 days year, 8 am to 8 pm Monday through Friday, and 8 am to 4 pm Saturday and Sunday.
