About Your Visit
Thank you for choosing FastMed! Prior to your visit, please read the following important information below.

At the time of registration, be prepared to provide the following:
- Photo ID
- Insurance card (if you have coverage)
- Co-pay, or self-pay fee if you are not paying with insurance
- FastMed Savings Plan Membership Card (click here for details) if applicable
Upon arriving at FastMed you will be greeted and asked for the items listed above and there may be additional important information to read and complete based on the reason for your visit. See the examples at the bottom of this page. We are committed to providing personal, quality care that is as convenient as possible.
Your wait time will be based upon the number of patients being treated before you and our triage (the degree of urgency based on the severity of the medical condition) of other patients. After check-in, our friendly staff will call your name, bring you to a private exam room and take your vital signs. When the provider sees you, they will discuss the reason for your visit, listen to your concerns, perform an examination, communicate their diagnosis and provide a treatment plan that is right for you. If any further testing is necessary or recommended, they will advise you on your options.
If there is a wait to be seen, ask your guest services specialist about FastMed’s CareCall Program. Available at select locations, with FastMed CallCall we will check you in and we will call you when we can treat you, allowing you to go home, back to work or any place you wish. After treatment and once your visit with our provider, you will be brought to the check-out area to receive a record of your visit and treatment discharge instructions.
After your visit, most visits patients will receive a follow-up phone call to check on your recovery, see if you have any questions and update you if there was any follow-up from your visit with FastMed.
Children (under age 18) must be accompanied by a parent or legal guardian. Any other accompanying adult must have a notarized statement signed by the parent/guardian authorizing treatment.

- If you need a flu shot you will need to complete a Vaccination Requirement and an Exemption form.
- If you need a sports or athletic physical, you will need to complete the pre-participation exam from.
- If you need treatment for a work place injury, you will need to complete a Worker’s Compensation Authorization form.
