At FastMed Urgent Care, we understand that if you're coming to see us, you're probably not feeling your best, so we strive to make your experience as convenient and efficient as possible. When you're a first time FastMed patient, you'll be asked to fill out a patient information form, so if you would find it more convenient to complete the form prior to coming in, please feel free to download the form here, fill it out as indicated and bring it with you. You'll be that much closer to the great care that our patients expect from our FastMed medical team.
