To qualify, follow a few simple steps:
If unable to complete steps 2 & 3 in advance, you can complete them in clinic and will be fully checked in for the sports physical only once all the necessary paperwork is done.
The timeframe begins when the patient is checked in to our system at the clinic, after the completion of all necessary paperwork (patients can e-register online to speed up this process). Completion of the sports physical is considered the time the patient is checked out upon discharge.
What is a sports physical?
Sports physicals are typically required to ensure your child’s safety while participating in organized sports. Our skilled medical professionals will conduct:
- A review of family and medical history
- Vital signs check
- Height and weight check
- Vision screening
- Vaccine review
FastMed follows CDC guidelines for a vaccine review. Patients must bring their vaccine history information in order for the vaccine review to be completed.

This exam is not a substitute for a comprehensive examination where important preventative information is covered.

What should I bring?
Bring any physical forms or documents required by the school or team, including sports physical or vaccine review forms. You will also need completed health and immunization documentation.
If your child requires glasses, sports goggles, corrective lenses or contacts to pass a vision test, please have these items with you at the time of your visit. Also, if your athlete has a history of heart problems, conditions, or episodes related to the heart, we require documentation from the athlete's primary care or treating physician. This must state that the condition has been evaluated and does not prevent participation in sports.
