FastMed Urgent Care offers all North Carolina students a coupon for a $25 sports physical at all FastMed locations for a limited time

August 7, 2012 – Starting August 1 through August 31, children and teens needing their required sports physical for 2012 fall and 2013 winter sports participation can get theirs for $25 when they visit a FastMed Urgent Care and present a FastMed coupon. The coupon, valid at all FastMed locations, is available for download HERE .
Sports Physicals are required annually for student athletes before participating in sports in North Carolina. The complete official form, available through the North Carolina High School Athletic Association, provides all the questions asked by the FastMed provider. The form is available on the NCHSAA website, www.nchsaa.org.
The questions the FastMed Provider will ask will include:
- Prior concussions or other head injuries
- Chronic medical illnesses such as diabetes and asthma
- Prior heart murmurs, heart infections or seizures
Parents and the student will receive a summary of the examination and the provider will stamp all required medical forms. If the physical results in the need for any further follow-up or lab tests, the provider will provide recommendations and options.
Insurance is not required to receive a sports physical, but payment is required at the time of service to receive the $25 physical examination for sports, normally $30 at all FastMed locations. The coupon must be presented to receive $5 off. FastMed Urgent Care has over 26 open locations throughout all of North Carolina communities.
