Simple Self-Pay Pricing

$179 initial consultation | $159 per month, ongoing care

Month-to-month with no long-term contract. Medication costs are separate. Urgent Care and other Primary Care visits are billed separately.


Who May Qualify?*

Your provider will confirm eligibility at your initial consultation. In general, the program is designed for adults who meet one of the following:

18+ Adults Only

The program is available to patients ages 18 years or older.

BMI 30+

Body Mass Index of 30 or Greater

Adults with a BMI of 30 or greater may qualify based on BMI alone.

BMI 27–29.9

Plus a Weight-Related Condition

  • Type 2 Diabetes
  • High cholesterol
  • Obstructive Sleep Apnea
  • Heart disease
  • Fatty liver disease

 

*Please note: This program is not available for patients who are pregnant, breastfeeding, or under age 18.


What Does the Program Include?

Two straightforward costs: a one-time initial consultation, then a monthly subscription for ongoing care and support.

Initial Consultation

$179 one-time, in-person visit

  • In-person visit with a healthcare provider
  • Review of your medical history and weight loss goals
  • Eligibility assessment for treatment options
  • Lab testing to support safe treatment planning
  • Personalized care recommendations

Ongoing Care & Support

$159 monthly subscription

 

  • Weight loss follow-up visits with your provider
  • Medication management, if appropriate
  • Progress monitoring and treatment adjustments
  • Ongoing clinical support
  • Virtual and in-person appointments
  • Annual laboratory monitoring after your first year

 

Urgent Care and other Primary Care visits are not included in the subscription and are billed separately.


Medications Available

If medication is appropriate for your treatment plan, your provider may prescribe one of the following GLP-1 therapies:

Semaglutide

Prescribed one month at a time. A provider visit — virtual visits are appropriate — is required to renew your prescription.

Tirzepatide

Prescribed one month at a time. A provider visit — virtual visits are appropriate — is required to renew your prescription.

 

Medication costs are separate from program fees. You’ll pick up your medication from your preferred pharmacy, and pricing will depend on your treatment plan and pharmacy.


Visit Expectations

A convenient combination of in-person and virtual visits, built around your schedule.

2 Follow-Ups per Month

Up to two follow-up visits per month, typically every two weeks.

1 In-Person Visit Every 3 Months

At least one in-person visit every three months for a physical evaluation.

$0 Additional Visits

Additional visits as needed at no additional charge while enrolled.

For best results, patients are encouraged to remain in the program for at least 6 months, although participation is month-to-month with no long-term contract.
